Vickie V. Brunt and her husband, the late James D. Brunt, on May 26, 1969, obtained an application form of the Standard Life Insurance Company of Jackson, Mississippi, from W. Roy Chaney, a vice president of the Clinton Bank and Trust Company, for a six-year level premium decreasing term insurance policy, more commonly known as mortgage cancellation insurance.
