The Department of Public Safety revoked the driver's license of Joseph Jolly Hayden because Hayden, allegedly, refused to submit to a blood-alcohol test upon request of an Owensboro police officer. KRS 186.565(3). The revocation subsequently was sustained at a hearing held by the Department of Public Safety pursuant to KRS 186.565(4).
