Claimant sustained a work related injury on 5 January 1964. The Industrial Commission assumed responsibility for claimant's back injury and also for his emotional problems related to the injury. On 7 November 1967, claimant, by his then attorney, filed an application for a lump-sum settlement although his attorney specifically advised against such application.
