After an automobile accident on June 22, 1968, Anthony Cuccia filed an insurance claim for personal injuries and property damage with Allstate Insurance Company under a policy that had been issued to him on October 4, 1967. He was told, however, that the policy had been cancelled on February 25, 1968, by written notice of cancellation mailed to him on February 12, 1968, at the address appearing on the policy.
