On November 6, 1971, Leonard Charles Brachett signed an application for a $16,000 life insurance policy with Universal Life Insurance Company. The agent, Peaches Marie Washington, collected at the same time $26.30 for the first quarterly premium and $5.00 "joining fee". The application was sent to the company's home office in Memphis, Tennessee, but before the issuance or delivery of a policy, Brachett died on November 21, 1971. Brachett's widow...
