Defendant, The Paul Revere Life Insurance Company (hereafter "the insurer"), issued to plaintiff, Morris Levin (hereafter "the insured"), an accident and health insurance policy sometime prior to October 22, 1959, the date on which insured was injured in an automobile accident.
