On December 8, 1958 Alene Heeter, hereinafter referred to as claimant, allegedly sustained injuries as the result of an accident which occurred on the premises of her employer, Moore Drug Company. Some 17 months later, on June 1, 1960 claimant submitted a Workman's Report of Injury to the Industrial Commission of Arizona.
