On April 8, 1963, The Franklin Life Insurance Company issued an insurance policy on the life of Charles Lester Ammons, aged 35, in the face amount of $100,000. The policy provided for double indemnity in the event "that the death of the insured resulted directly and independently of all other causes, from bodily injuries effected solely through external, violent and accidental means."
