Two questions are presented for determination in the instant case: (1) Was the employer's notice of appeal from the decision of the Industrial Commission, as quoted above, legally sufficient? (2) If so, did the claimant's failure to file, within 30 days after the filing of the notice of appeal, a petition "setting forth the basis of the jurisdiction of the court over the action and setting forth the issues" in accordance with Section 4123.519, Revised Code...
