The petitioner, a naturalized citizen of the United States, applies for an order commanding the Board of Elections in the City of New York and the inspectors of election in the election district in which he desires to effect his permanent registration to permit him so to register without executing the form prescribed by the Board of Elections and called "naturalization affidavit".
