On June 17, 1953, Otto Quinton Casteel while in the employ of Thurston Chemical Company sustained an accidental injury consisting of an injury to his head resulting in his death.

On July 16, 1953, Madge Oleta Casteel, surviving widow of deceased, individually and as next friend of three minor children, dependents and heirs of deceased, filed a claim for compensation under the Death Benefits Provision of the Workmen's Compensation Law against...
