{¶1} Cassandra Lackey, a mentally-disabled adult, choked to death while eating in the cafeteria of the Medina County Board of Developmental Disabilities Achievement Center. Although Ms. Lackey was supposed to be on a mechanical soft diet, that fact was never communicated to cafeteria staff.
