Albert G. Abena worked as a dentist for American Dental Partners, Inc. ("American Dental") from 1993 until he became disabled in 2000. He applied for long term disability benefits under a plan sponsored by his employer and administered by Metropolitan Life Insurance Co. ("MetLife"), which was also the plan fiduciary.
