On March 5, 1994, defendant Gregory L. Henry was arrested for operating a motor vehicle under the influence of alcohol in violation of R.C. 4511.19(A)(1). Defendant was read a copy of the form required by R.C. 4511.191 and agreed to submit to a blood test.
