The petitioner, the Assistant Secretary of State of the State of Arizona, was on official business in Prescott on 11 May 1963. While attending a meeting in a hotel dining room a large staff bearing a flag fell and struck her on the neck and head. Her claim was not filed with The Industrial Commission until 1 December 1966 at which time she executed an election to take compensation.
