This is an appeal from an order and award entered by the full Industrial Board of Indiana (hereinafter referred to as the "Board"). The Board decided that the plaintiff take nothing by her Form 9, Application of Injured Employee to the Industrial Board for the Adjustment of Claim for Compensation.

After evidence was heard by a single member of the Board, the examiner entered an order against petitioner. She thereupon filed a Petition for Review...
